A fire accident on one of the shooting sets introduces Shanti to Om. As Om finds happiness and satisfaction in coming to Shanti's rescue, he is shattered upon discovering that Shanti is secretly married to film producer Mukesh Mehra (Arjun Rampal), a heartless opportunist, who kills her after she admits her pregnancy to him. As a witness to this crime, Om gets killed in a fight with Mehra's men. Om Prakash is reborn now as Om Kapoor, a star-kid. When flashes of his past-life are revealed to him in visions, Om Kapoor focuses all his energies on taking revenge on Mehra for killing his love, Shanti. He plans a conspiracy with Shanti's clone Sandy, his mother and friend, Papu to make Mehra (who is now a big producer in Hollywood) confess about his deadly crime. Trivia:
The film features cameo appearances from a good portion of the Bollywood industry, with most of them appearing as themselves.

In the song "Dastaan-E-Om Shanti Om", Shanti's ghost can be seen on a balcony behind Mike, just after he chased Sandy for the first time. Shanti's ghost is in the same costume she was wearing when she died.

Director Farah Khan wanted to bring Sanjay Dutt, Shah Rukh Khan, Aamir Khan, Salman Khan and Saif Ali Khan together onscreen for the first time ever. However, Aamir Khan refused to make a cameo appearance.

The yesteryear cars used in the film once belonged to Rajesh Khanna and Hema Malini.
